Why Falling in Love Can Feel Like an Addiction — What the Science Actually Shows

If you've ever described a new relationship as "intoxicating," you were closer to the science than you might think. A 2025 theoretical review in the journal Cognitive, Affective, & Behavioral Neuroscience, by researchers Tobias Esch and George Stefano, takes a close look at why romantic love and drug addiction light up such strikingly similar circuits in the brain — and offers a new hypothesis about what might be happening at the cellular level to explain it.

Here's what the article says, and just as important, what it doesn't prove.

The shared brain circuitry of love and drugs

Both falling in love and using addictive drugs activate what's called the brain's reward pathway, a network running from the ventral tegmental area through the nucleus accumbens, with input from the prefrontal cortex, hippocampus, and amygdala. This circuit runs mainly on dopamine, the neurotransmitter tied to motivation and pleasure.

Brain imaging studies cited in the review found that people who describe themselves as intensely "in love" show activation in the same dopamine-rich regions that light up during drug use. Interestingly, even long-term couples who still report being "madly in love" show sustained activity in these reward pathways, alongside increased activity in areas rich in opioid and oxytocin receptors, suggesting a shift from early infatuation toward a calmer, more secure form of attachment over time.

The review also touches on the flip side: what happens when love ends. Grief and heartbreak activate some of the same brain regions involved in physical pain. The authors point to "broken heart syndrome" — a real, temporary heart condition triggered by acute emotional stress, as a striking example of how emotional pain can produce measurable physical effects.

The mitochondria connection

The more novel part of this article is its focus on mitochondria, the energy-producing structures inside our cells. The authors propose that practices which trigger the "relaxation response" (slow breathing, meditation, and similar mind-body techniques) may improve how efficiently mitochondria produce cellular energy (ATP), partly through a molecule called nitric oxide.

Their hypothesis: stress and addiction disrupt this cellular energy balance, while love, bonding, and relaxation-based practices may help restore it. If true, this would help explain why relaxation techniques sometimes ease withdrawal symptoms in people recovering from substance use, and why strong relationships tend to correlate with better overall health.

A critical look: what to keep in mind

This is a genuinely interesting and readable article, but it's important to understand what kind of paper this is before drawing conclusions from it:

·        It's a theoretical review, not a new study. The authors didn't collect any new patient data. Instead, they pulled together findings from many other studies to build a hypothesis. That hypothesis — that nitric oxide-driven mitochondrial changes explain the benefits of relaxation and bonding — is thoughtful, but it remains unproven and is explicitly labeled a hypothesis by the authors themselves.

·        Heavy reliance on the authors' own prior work. A large share of the citations supporting the central mitochondrial hypothesis trace back to earlier papers by these same two researchers. That's not necessarily a red flag on its own, but it does mean the idea hasn't yet been independently replicated by other labs in the way the underlying dopamine-reward findings have.

·        The dopamine/reward-pathway science is well established; the mitochondrial piece is speculative. The parts of the article describing shared brain reward circuitry between love and addiction rest on a solid, decades-deep body of neuroimaging research. The newer claims about nitric oxide and mitochondrial "energy optimization" are conceptually interesting but are, by the authors' own admission, a working model that needs future research to confirm.

·        No conflicts of interest were declared, and the paper is open-access, which is worth noting positively for transparency.

What this means for you

The take-home point isn't "love is a drug" in some alarming sense — it's that our brains are wired to find deep social bonds intensely rewarding, using much of the same biological machinery that drugs of abuse hijack. That's actually reassuring it suggests that healthy sources of connection and calm — real relationships, mindfulness practice, slow breathing — may support the same reward and stress systems that addiction disrupts, without the downsides.

If you're working through anxiety, stress, or recovery from substance use, this research adds to a growing body of evidence that mind-body practices like breathwork and meditation aren't just "nice to have” they may have a real, biological basis for helping regulate your nervous system. That said, they're best used as part of a broader, individualized treatment plan rather than a standalone fix.
